Insurance Information
Our goal is to provide outstanding comprehensive, personalized care that is complimented by superior customer service. As a courtesy to all our patients, we will submit claims to your primary and secondary insurance carriers. It is impossible for us to know all the different policies for individual insurance companies, but we will try our best to inform you of your outpatient physical therapy benefits and to keep you informed of any limitations of services. Most insurance carriers will require a written prescription from a physician in order for your care to be covered.
